One woman died. The record says she chose not to go to hospital. Her daughter says that is not what happened. When Rachel Vale comes to Threshold with three copies of the same document, Samira Cruz sees a familiar problem: an institution has produced a clean, confident account of a night that was anything but clean. Marian Vale, sixty-eight, was recovering at Ashdown after surgery when her condition worsened. A hospital transfer was discussed. A booking was made. Then the transfer was cancelled. By the next morning, Marian was dead. Ashdown's final summary says Marian declined hospital transfer because she preferred to remain unless it was clinically essential. It is plausible. It is orderly. It may even contain pieces of the truth. But the timing does not fit. A call to Rachel suggests the transfer was already being arranged. A separate transport record shows it was accepted and then cancelled before the first surviving note states that Marian wished to stay. Somewhere between clinical judgement, staffing pressure, operational decisions and retrospective review, a complicated sequence has been compressed into a simple official version: the patient chose. Samira's job is not to investigate a crime. Threshold exists to reconcile records, not to prove guilt. With Dan tracing the practical reality behind the paperwork and Detective Marcus Bell waiting beyond the evidentiary line Threshold cannot cross, Samira begins reconstructing Marian's final evening one decision at a time. Every timestamp matters. Every handoff matters. So does the difference between a temporary operational pause, a clinical judgement and a patient's expressed preference. The closer the team gets to the sequence itself, the clearer it becomes that the final summary has joined separate events together until cause and responsibility are almost impossible to see. Who authorised the hold? Why was the transfer not restarted? When did Marian's reluctance become a refusal? And who benefited when the record made those questions disappear? The deeper Samira goes, the more dangerous the simplicity of the official account becomes. Not because every part of it is false, but because true fragments can be arranged until responsibility seems to belong to someone who never made the crucial decision. For Rachel, the stakes are painfully personal. Her brother has spent months believing their mother died after getting what she wanted. Rachel needs to know whether Marian truly chose to stay—or whether an institution placed that choice in her mouth after the fact. For Samira, the case tests Threshold's defining boundary. Reconciliation can expose contradiction, recover sequence and restore agency. But when a record has been shaped not only by error and assumption, but by self-protection, reconciliation may no longer be enough. The Official Version is a tense, intelligent procedural thriller about institutional truth, medical uncertainty, responsibility and the quiet power of a record that looks complete. It is a story about what happens when no single lie creates the problem—only a chain of defensible decisions, incomplete records and one version that becomes official because it is easier to live with. Book Four in The Postmortem Archive series, featuring Samira Cruz, Dan and Detective Marcus Bell. It can be read as part of the series or as a standalone case.
