The book is organized around four categories of intellectual failure: the foundation of clear thought, the quality of evidence, the direction of attention, and the character of the thinker. The fifth chapter examines the accident at Three Mile Island —a catastrophic event where the disaster stopped—as a combination of all 15 standards under extreme pressure.
